What Is Adrenal Fatigue?

Adrenal fatigue refers to a functional imbalance of the adrenal glands, where they struggle to produce adequate levels of stress hormones — particularly cortisol.

While adrenal fatigue is not considered a formal medical diagnosis, it is widely recognised in functional and nutritional medicine as a contributor to ongoing fatigue, poor stress tolerance, disrupted sleep, and hormone imbalance.


Symptoms of Adrenal Fatigue

Common adrenal fatigue symptoms include:

  • Persistent low energy
  • Feeling exhausted upon waking
  • Difficulty handling stress
  • Feeling overwhelmed or “burnt out”
  • Low blood sugar symptoms
  • Brain fog and poor concentration
  • Mild anxiety or low mood
  • Cravings for caffeine, sugar, or salt
  • Frequent colds or low immunity
  • Digestive problems
  • Low body temperature
  • Increased abdominal fat

The Role of the Adrenal Glands

The two adrenal glands sit above the kidneys and secrete over 50 hormones that regulate:

  • Energy production
  • Blood sugar balance
  • Stress response
  • Immune function
  • Blood pressure
  • Fluid and electrolyte balance
  • Sex hormone production

Cortisol and Energy Regulation

Cortisol is one of the most important adrenal hormones. Its functions include:

  • Regulating blood glucose
  • Mobilising fat and protein for energy
  • Supporting blood pressure and heart rate
  • Influencing brain function and immune response

Cortisol Rhythm and Adrenal Fatigue

Cortisol follows a daily circadian rhythm:

  • Lowest: midnight – 4am
  • Highest: 6am – 8am
  • Gradually declines throughout the day

In adrenal fatigue, this rhythm becomes disrupted, often leading to:

  • Morning exhaustion
  • Afternoon energy crashes
  • A “second wind” late at night

Typical Adrenal Fatigue Energy Pattern

  • Morning fatigue
  • Slight improvement after midday
  • Energy dip between 2–4pm
  • Improvement after 6pm
  • Wired but tired feeling after 11pm

What Causes Adrenal Fatigue?

Adrenal fatigue develops due to chronic stress exposure, including:

  • Psychological stress (work, family, emotional strain)
  • Over-exercising or under-recovering
  • Poor diet and nutrient deficiencies
  • Blood sugar instability
  • Excess caffeine, alcohol, or sugar
  • Chronic inflammation
  • Poor sleep quality
  • Digestive dysfunction

Over time, the constant demand for stress hormones leaves the adrenals unable to keep up.


Testing for Adrenal Function

Conventional medicine often uses blood cortisol tests to rule out conditions such as Cushing’s Syndrome or Addison’s Disease.

In functional medicine, we use a salivary Adrenal Stress Profile, which measures cortisol levels at multiple points throughout the day. This provides a far more accurate picture of daily adrenal function and hormone patterns.

How to Treat Adrenal Fatigue Naturally

1. Improve Your Diet

Nutrition is foundational to adrenal recovery.

Key principles:

  • Eat small, regular meals
  • Balance protein, fats, and complex carbohydrates
  • Avoid blood sugar spikes

Focus on:

  • Lean proteins
  • Whole grains
  • Healthy fats
  • Plenty of vegetables

Avoid:

  • Refined sugar
  • Ultra-processed foods
  • Excess caffeine and alcohol
Good food choices can improve adrenal fatigue.

2. Improve Sleep Quality

Sleep is critical for adrenal repair.

  • Cortisol should be lowest overnight
  • Disrupted rhythms cause poor sleep and morning fatigue

Tips:

  • Create a calming bedtime routine
  • Avoid screens close to bedtime
  • Try meditation, breathing exercises, gentle yoga, or reading
Good sleep is important in recovering from adrenal fatigue.

3. Balance Blood Sugar Before Bed

Low cortisol can lead to night-time blood sugar dips, waking you from sleep.

A small bedtime snack may help, such as:

  • Avocado on a wholegrain cracker
  • A boiled egg
  • Mixed nuts
  • Wholegrain toast with nut butter
